REQUEST A FIRE REPORT

You can request copies of a Fire Report from Martin County Fire Rescue. Reports are Public Record. Click the "Request a Fire Report" button below to submit a request for a fire report. Please provide the date and address of the fire. Staff will respond whether the report is ready to be released.

The report can be picked up at Fire Rescue Administration located at 800 SE Monterey Road in Stuart. There is a charge of $.15 per single sided or $.20 per double sided. The report can also be emailed at no charge. Requests for reports can also be made by calling (772) 288-5710 or (772) 288-5633.

If there is a fire investigation, then that can take an extended period of time for the Fire Inspector’s report to be complete. When the State Fire Marshal’s Office is investigating the fire, you will need to contact them for an estimate of when the report will be complete and if there are any charges. REQUEST MEDICAL RECORDS 

Every person treated by Martin County Fire Rescue is entitled to their Medical Records; these reports are Public Record. To obtain medical records please complete the Authorization for Release of Protected Health Information (PHI) Form, then attach it to your request via the button below. The authorization form may also be picked up at Fire Rescue Administration located at 800 SE Monterey Road in Stuart, or it may be sent via email.

All authorizations must be properly filled out and notarized. If the requestor would like to come in person to request their own records, they must bring a valid photo ID to affirm one’s identity. All requests are processed in the order in which they are received, so it may be several weeks to receive your records.
